How to Submit

Who can enter: Anyone impacted by FTD—care partners, family members, friends, and those living with a diagnosis.

What you’ll need:

1–2 high-quality photos of your pet (JPEG/PNG)
A short story (max 250 words) answering one of these prompts:
How did your pet support you or your loved one during the FTD journey?

What makes your pet special to your family?

A favorite memory involving your pet and caregiving/life with FTD.

Limit: 1 entry per household

meet the Artist:

Alyssa Nash

Alyssa Nash is a 29-year-old multidisciplinary artist based in Iowa whose work—spanning oil painting, calligraphy, and poetry—explores themes of grief, identity, resilience, and healing. Her art speaks to the soul, blending vulnerability and strength across every medium she touches.

Art has always been part of Alyssa’s life. As a child, creating helped her process the early stages of her father’s journey with Frontotemporal Dementia (FTD). She earned an art scholarship to the University of Northern Iowa, where she studied Art History, but after learning in 2016 that she carries the same FTD gene, she began to struggle and eventually stepped away from both school and her creative practice.

Years later, after focusing on self- connection, Alyssa found her way back to art. It became a lifeline—a space to create, hope, and heal. Now, her work serves as both personal expression and advocacy, offering comfort to others navigating similar paths. Her art was recently featured at the Association for Frontotemporal Degeneration’s (AFTD) 2025 Educational Conference, where she shared her story as both a caregiver and gene carrier.

Alyssa’s beloved dog, Nala, has been a constant source of support throughout her journey. She is proud to honor Nala—and all “furry caregivers”—in the upcoming Go-Mo Creatives 2026 Pets of FTD Calendar.

Go-Mo Creatives (“we,” “our,” or “us”) is committed to protecting your privacy. This Privacy Policy explains how we collect, use, and protect the personal information you provide in connection with the 2026 Pets of FTD Calendar Campaign, hosted via Shortstack.

By submitting your information, you consent to the terms of this Privacy Policy.

1. Information We Collect

When you participate in the campaign, we may collect the following:

  • Your first name and U.S. state (for calendar credits)

  • Your social media handle (optional)

  • Your email address (for winner notification)

  • Photos and written content you upload as part of your entry

  • IP address and browser data (automatically collected by Shortstack)

2. How We Use Your Information

We use the information you provide to:

  • Administer the calendar campaign

  • Contact you if selected as a semi-finalist or winner

  • Publicly credit you if your submission is featured (first name, state, and optional social handle)

  • Promote awareness of FTD through the calendar and related materials

  • Improve our campaigns and community engagement efforts

We will never sell, rent, or trade your personal information.

3. How We Share Your Information

Your submission (pet photo, story, and name/state/handle) may be published in:

  • The printed and POD versions of the 2026 Pets of FTD Calendar

  • Social media and online marketing promoting the campaign

  • Media or advocacy materials related to FTD awareness

We do not share your email or contact information publicly. Data submitted through Shortstack is also subject to Shortstack's Privacy Policy.

4. Data Security

We implement reasonable safeguards to protect your data during the campaign. However, no data transmission or storage can be guaranteed 100% secure. By participating, you acknowledge this risk.

5. Retention

We will retain entry data (photos, text, contact details) for the duration of the campaign and for record-keeping, promotional use, or historical archiving. You may request removal of your personal data by contacting us at sisters@gomocreatives.com.

6. Your Rights

You may request:

  • A copy of the data we’ve collected from you

  • Correction of inaccurate information

  • Deletion of your personal data (which may remove you from eligibility)

7. Children's Privacy

This campaign is not intended for children under 13, and we do not knowingly collect personal data from anyone under 13.
